Indexes - Generation and Sorting.

The index is generated at the end of the
processing and is sorted alphabetically
before being appended to the end of the
document file.

Upper and lower case letters are regarded as
the same for sorting purposes, and the actual
entries have the first letter capitalized if
this is not done in the indexing command.
